The reader wants the code reasonably centered, sharp, and with a bit of the light quiet zone around it intact; heavy motion blur, extreme rotation, glare, or a photo cropped tight against the code's edge are the usual reasons a scan fails. Re-crop or brighten the picture and try again before concluding the code is broken.
